New Patients
If you would like to become a patient of Hays Family Medicine, please fill out the New Patient Request form and submit this to us. We ask that you do this so we can try to understand your medical situation and determine whether we have a provider with the expertise to properly care for you. You should hear back from us in 2-3 business days. If you have not been contacted, please call us. Once you have been accepted as a new patient, we will have you go to the Patient Forms link to provide us with your medical history and other information. Please bring your current insurance card to your visit. Your insurance co-pay is due at the time of service. We accept VISA, Discover, Master Card, check or cash. We hope you will let us provide care for you and your family!
