It is our office policy to notify the provider you are currently seeing that you request to change providers. We will try to accommodate your request if possible.

You will need your insurance card, medication list, list of health problems and past surgeries. A list of your allergies and family history is also important to bring. Your insurance co-pay is due at the time of service. We accept VISA, Discover, Master Card, check or cash.

Our clinic is open from 8-5 M-TH and 8-4 on Friday. Feel free to call us for your appointment request.

We always appreciate a phone call to notify us if you cannot make your appointment. It allows us to care for other patients that may be ill in your appointment time. If you accidentally forget, please call our office to reschedule. Missing appointments without notifying us makes it difficult for us to adequately treat you. It also makes other patients wait for an appointment that could have gotten your appointment time.

Our physicians rotate working at the hospital each week. If your particular physician is not at the hospital, know that a family medicine physician will be caring for you. Be assured that the physician at the hospital will communicate with your primary physician in order to best care for you while you are in the hospital.
